• 1. Email the forms to DDDApply@azdes.gov. (This is the fastest way to process your application)
• 2. Mail the forms to any DDD office statewide
• 3. Drop off the forms to any DDD office statewide

• Division of Developmental Disabilities (DDD) provides support services (like habilitation, day programs) for people with developmental disabilities.
• Arizona Long Term Care System (ALTCS) is the Medicaid funding source that pays for long-term, intensive care services, often acting as a “scholarship” for those in DDD. Without ALTCS DDD services will have to be paid out of pocket.
