How to Apply for DDD Services?

You can submit the application and required documentation in 1 of 3 ways:
- 1. Email the forms to DDDApply@azdes.gov. (This is the fastest way to process your application)
- 2. Mail the forms to any DDD office statewide
- 3. Drop off the forms to any DDD office statewide

Why It Is Important to Have Both ALTCS & DDD?
- Division of Developmental Disabilities (DDD) provides support services (like habilitation, day programs) for people with developmental disabilities.
- Arizona Long Term Care System (ALTCS) is the Medicaid funding source that pays for long-term, intensive care services, often acting as a “scholarship” for those in DDD. Without ALTCS DDD services will have to be paid out of pocket.

- Send your loved ones most recent PCSP
- Send our agency your loved ones service hours (attendant care, habilitation, IDLA, DTA/TRA, and/or respite.)
